Job Summary: As a Nurse Care Manager, you'll provide support and activities to clients through an organized treatment plan designed to ease an individual's immediate and continued adjustment to community living. You'll be working in the community, including clients' homes, clinics, and other places around the community as part of your regular assigned caseload. Our ideal candidate is passionate about promoting recovery and increased resiliency, has experience with evidence-based interventions, and strong communication and collaboration skills.

Key Responsibilities:
Collaborate with the interdisciplinary team to create individualized treatment plans for clients with co-occurring chronic diseases
Develop and implement wellness and prevention initiatives
Facilitate health education groups on complex medical conditions
Manage hospital admissions/discharges for Healthcare Home patients
Train patients on medical diagnoses, treatments, medications
Implement DMH technology programs like CyberAccess, metabolic screening
Monitor and report performance measures and outcomes
Demonstrate excellent written and verbal communication skills
Utilize critical thinking and nursing process for informed decisions
Requirements:
Active RN or LPN license
Direct patient care experience preferred
Strong understanding of chronic disease processes
Excellent communication and interpersonal skills
Ability to adapt to new technologies
Critical thinking and problem-solving skills
Passion for exceptional patient care
